| Notes | Please fill out this form to apply. https://business.form-mailer.jp/fms/ec84c682230418 If the form is not available, please download the application form, fill it out, and send it to us by e-mail (fax or mail is also acceptable). 【Review Process】 Membership applications are reviewed and approved at the Board Meetings (scheduled to be held four times per year: around January, May, September and December). Upon approval of the application, an invoice of the membership fee is sent to the applicant. 【Student Membership】 Please attach the copy of your Student ID to the application form for Student Membership. Student membership is open to those who are majoring in toxicologic pathology or its related fields on undergraduate courses or in Master's or Doctoral degree programs at graduate schools (Article 7 of the Constitution of JSTP). After graduation or completion of the degree programs, please submit the application to change the type of membership to Full Membership (Membership Type Change). 【Application Form】 Please make sure to fill all the items in the application form. Please list on separate sheets in case of plural.If you do not have a list of your works, please indicate as "None". A sponsor (JSTP Councilor or Board Certified Toxicologic Pathologist) must sign and seal your application form. If you have difficulty in finding a sponsor for your membership application, please contact JSTP Secretariat. |
